Natural Breast Augmentation Without Implants—Using Your Own Body Fat
Many women want fuller breasts but don’t want any foreign material in their bodies. For these patients in particular, breast augmentation with autologous fat is the ideal solution. The result not only feels natural—it is natural. After all, what I use is already part of your body.
I am Dr. Juliane Bodo, a board-certified specialist in plastic and aesthetic surgery in Berlin-Charlottenburg. Breast augmentation using the patient’s own fat is one of the procedures I particularly enjoy discussing with patients—because it fulfills two desires at once: more volume in the breasts and less fat in another area of the body.
Who is a good candidate for breast augmentation using autologous fat?
This method is the right choice if you:
- are looking for a gentle, natural breast augmentation of up to one cup size
- would like to avoid silicone implants
- have enough body fat on the abdomen, hips, or thighs
- and would like to benefit from body contouring at the same time
- prefer a shorter recovery time
Important to know: Approximately 500 ccm of fat tissue is needed per breast. You don’t need to gain weight for this—the fat can be harvested from various parts of your body. During the consultation, we’ll work with you to determine whether there is enough suitable fat tissue available.
How does breast augmentation using the patient's own fat work?
The surgery is performed under general anesthesia and lasts about 1.5 hours. It is an outpatient procedure—you can go home the same day.
Step 1 – Fat Removal I gently remove fat from the desired area of the body—such as the abdomen, hips, or thighs—using water-jet-assisted liposuction. This procedure results in a particularly high survival rate for fat cells. The incisions are tiny—the scars are barely visible.
Step 2 – Fat Processing The harvested fat is purified and processed in a special lipocollector. This step is crucial: Only viable, intact fat cells are later transplanted.
Step 3 – Insertion into the breast The processed fat is injected into the breast in several layers using a fine cannula. The glandular tissue remains completely unaffected—the ability to breastfeed is preserved.
What You Should Know Before Surgery
A mammogram is required before the procedure—to ensure that there are no abnormal tissue changes. Another mammogram one year after the procedure is also recommended. We’ll discuss this together during the consultation.
Benefits of Breast Augmentation with Autologous Fat
- Completely natural results—no foreign bodies, no risk of capsular fibrosis
- A dual aesthetic benefit: more breast volume and less fat at the donor site
- Very small scars—about 3 mm at the donor sites
- Shorter recovery time than with implant surgery
- Socially active immediately; can resume sports after 2 weeks
Things to Keep in Mind
Breast augmentation using the patient’s own fat is technically more challenging than implant surgery—and the achievable increase in size is limited to about one cup size. Since fat cells are delicate and some may be broken down by the body over time, two procedures are usually necessary to ensure a lasting result. You should take this into account when planning your budget.
During our consultation, I’ll tell you openly and honestly whether this method is right for you—or whether another option is a better fit for your needs and your current situation.
Risks & Potential Complications
As with any surgical procedure, complications can also occur with breast augmentation using the patient's own fat.
- Fat Resorption: Over time, the body may break down some of the injected fat. For this reason, two procedures are usually necessary to ensure long-lasting results.
- Fat necrosis: In rare cases, fat cells may die and form small hardened areas in the tissue. These are generally harmless but can raise questions during imaging exams—which is why a mammogram is required both before and after the procedure.
- Asymmetries: Since fat accumulates at different rates in each breast, slight differences in volume may occur. These can be corrected during a second procedure.
- Swelling & bruising: Both at the extraction site and on the breast—these will subside within the first few weeks.
- Infections: Very rare when sterile surgical techniques are used and follow-up care is consistent.
- General surgical risks: reactions to anesthesia, thrombosis—these will be discussed in detail during your personal consultation.
I will discuss all risks with you in detail before the surgery. A mammogram is required before the procedure and one year afterward—for your safety.
